Support GUID-less PDBs (#77). r=bryner

- Handle MDCVInfoPDB20-based PDBs by outputting a signature instead of a guid
   in the MODULE line.
 - Identify the OS and CPU in the MODULE line.
 - Suppress multiple subsequent identical STACK WIN lines.
