i386: Update new x86_apicid parsing rules with die_offset support
In new sockets/dies/cores/threads model, the apicid of logical cpu could
imply die level info of guest cpu topology thus x86_apicid_from_cpu_idx()
need to be refactored with #dies value, so does apicid_*_offset().

To keep semantic compatibility, the legacy pkg_offset which helps to
generate CPUIDs such as 0x3 for L3 cache should be mapping to die_offset.

target/i386: Use env_cpu, env_archcpu
Cleanup in the boilerplate that each target must define.
Replace x86_env_get_cpu with env_archcpu. The combination
CPU(x86_env_get_cpu) should have used ENV_GET_CPU to begin;
use env_cpu now.

pc: use generic cpu_model parsing
define default CPU type in generic way in pc_machine_class_init()
and let common machine code to handle cpu_model parsing

Patch also introduces TARGET_DEFAULT_CPU_TYPE define for 2 purposes:
* make foo_machine_class_init() look uniform on every target
* use define in [bsd|linux]-user targets to pick default
cpu type

i386: keep cpu_model field in MachineState uptodate
Considering that features are converted to global properties and
global properties are automatically applied to every new instance
of created CPU (at object_new() time), there is no point in
parsing cpu_model string every time a CPU created. So move
parsing outside CPU creation loop and do it only once.

Parsing also should be done before any CPU is created so that
features would affect the first CPU a well.

pc: Generate init functions with a macro
All pc-i440fx and pc-q35 init functions simply call the corresponding
compat function and then call the main init function. Use a macro to
generate that code.

machine: Remove unused fields from QEMUMachine
This removes the following fields from QEMUMachine: family, alias,
reset, hot_add_cpu, units_per_default_bus, no_serial, no_parallel,
use_virtcon, use_sclp, no_floppy, no_cdrom, default_display,
compat_props, and hw_version.

The only users of those fields were already converted to use QOM and
MachineClass directly, so they are not needed anymore.

pc: Don't use QEMUMachine anymore
Now that we have a DEFINE_PC_MACHINE helper macro that just requires an
initialization function, it is trivial to convert them to register a QOM
machine class directly, instead of using QEMUMachine.

Include less of the generated modular QAPI headers
In my "build everything" tree, a change to the types in
qapi-schema.json triggers a recompile of about 4800 out of 5100
objects.

The previous commit split up qmp-commands.h, qmp-event.h, qmp-visit.h,
qapi-types.h. Each of these headers still includes all its shards.
Reduce compile time by including just the shards we actually need.

To illustrate the benefits: adding a type to qapi/migration.json now
recompiles some 2300 instead of 4800 objects. The next commit will
improve it further.

Include qapi/error.h exactly where needed
This cleanup makes the number of objects depending on qapi/error.h
drop from 1910 (out of 4743) to 1612 in my "build everything" tree.

While there, separate #include from file comment with a blank line,
and drop a useless comment on why qemu/osdep.h is included first.

i386: Remove AMD feature flag aliases from Opteron models
When CPU vendor is set to AMD, the AMD feature alias bits on
CPUID[0x80000001].EDX are already automatically copied from CPUID[1].EDX
on x86_cpu_realizefn(). When CPU vendor is Intel, those bits are
reserved and should be zero. On either case, those bits shouldn't be set
in the CPU model table.

Commit 726a8ff68677d8d5fba17eb0ffb85076bfb598dc removed those
bits from most CPU models, but the Opteron_* entries still have
them. Remove the alias bits from Opteron_* too.

Add an assert() to x86_register_cpudef_type() to ensure we don't
make the same mistake again.

target-i386: Move APIC ID compatibility code to pc.c
The APIC ID compatibility code is required only for PC, and now that
x86_cpu_initfn() doesn't use x86_cpu_apic_id_from_index() anymore, that
code can be moved to pc.c.
